Admissions Checklist

The applicant's parents/guardians are responsible for making certain that all necessary application forms are completed and returned to the Admissions Office in a timely manner. Please use the following Admissions Checklist to manage your application to The Swift School. You may print this checklist below.

Submit current Psychoeducational Report.

Submit Authorization to Release Student Records to applicant's current and past schools. Please follow up to ensure these forms are completed and mailed.

Submit Confidential Teacher Evaluation Form to applicant's current teacher(s). Additional copies of this form may be printed from Admission Forms. Please follow up to ensure this form is completed and mailed.
